Dear Mr. Keeler:
A plumbing pemut was issued on December 12, 1999 to install a water heater at your address.
According to section 4715.2800 of the State Plumbing Code, a final inspection shall be conducted
on the plumbing authorized by this permit. As of this date, no inspection has been called for.
The pernut fee paid covers the inspector's time to make sure the appliance was installed
correctly. We also check to make sure combustion air has been installed and that the vent and
vent connectors have been sized properly.
You will be given thirty (30) days from receipt of this notice to respond. If we do not hear from
you, we will expire the permit and any further inspection on this installation will require a new
pertnit.

A plumbing permit was issued on July 2, 2003 to insta.11 a water softener at your address.
The final inspection was conducted on July 9, 2003 and it was noted that the following
conection needed to be completed and a reinspection called for;
Provide a minunum 1-1/2" air gap into laundry tub per manufachuers instructions
pages 3-5.
We ha.ve not been contacted to reinspect the conections. We will keep your permit open
for another 30 days and if we do not hear from you within this tune to set up the
inspection, the permit will be marked "no inspection called for by homeowner after
notification" and expire the permit.
