While acting on a complaint, an inspection was conducted on March 24, 1995 at
5210 Buchanan Street NE, Fridley.
My investigation has determined that the homeowner contacted your firm to
purchase and install a water heater. The homeowner has allowed me to copy
the receipts of this purchase (copies attached). The gas utility,
Minnegasco, was contacted on March 20, 1995 for a gas smell at the water
heater. My inspection has determined that the following corrections are
necessary to be in compliance with the manufacturer's listing;
1) Outside combustion air shall be provided per Chapter 6, 1991 UMC and
Section 504(A) 1991 UMC.
2) Pipe on temperature and pressure relief valve shall extend not less
than 18 inches from the floor per MN Rules 4715.2210.
3) Gas shut-off valve shall be of an approved type per 1991 UMC Section
503(A).
4) The required permit was not obtained per 1991 UMC Section 301(A).
All corrections shall be performed by an approved agency within 14 days of
this notice.

Dear Ms. Ungemach:
A mechanical permit was issued on October 19, 2000 for the installation of a gas
furnace for your atldress. According to Section 305(a) of the 1991 Uniform Mechanical
Code, a final inspection shall be performed on the mechanical permit. As of this date no
inspection has been called for.
The permit fee that was paid covers the inspector's time to check to make sure the
furnace was installed correctly. We also need to check for the installation of combustion
air, vent and connector sizing.
You will be given thirty (30) days from receipt of this notice to respond. If we do not
hear from you, your permit will expire and any inspections on this installation will require
a new permit.
