THE FIRE OR LIFE SAFETY CONDITIONS STIPULATED BELOW ARE REQUIRED
DURING THE PERFORMANCE OF THE PERMITTED ACTIVITY
Hazardous materials to be used as fumigants include:

1. All pexmitted operations and activities shall comply with all applicable provisions contained in 2007 MSFC
Chapter 17, Fumigation and Thermal Insecricide Fogging,with special attention to those listed below.
2. Warning signs shall be posted as required by 2007 MSFC Section 1703.3.1. In addition,waxning signs shall
contain the language and be prepared as shown in pxoduct label information and posted at all entrances as
Yequired by that same information. Because product specific information concerning precautions including
entry to the building by personnel without proper personal protective equipment shall be enforced by
contractor and owner.
3. Permittee shall abide by product specific personal pxotection equipment required by MSFC,MSDS and
product labeling. 2007 MSFC 1703.3.2 also requixes contractox to maintain and have available appxoved
protective bxeathing appaxatus for use by woxkers.
4. Contractor shall provide capable, alert watch personnel duxing the period fumigation process on duty at the
entrance or entrances to the enclosed fumigation spaces until after the fumiga.tion is completed and
premises properly ventilated and safe fox occupancy. Sufficient watchers shall be provided to prevent
persons from entering the enclosed space under fuinigation without being observed. (2007 MSFC 1703.3.3)
5. At the end of the exposuxe period, conttactor shall safely and properly ventilate fumigated portions of
premise and contents, properly dispose of fumigant containers,residues, debris and other materials used for
fumigation; and cleax any obstructions from gas-fixed appliance vents,if used.
6. Permittee shall also notify Fridley Fire Departrnent Dispatch Center when fumigation begins and when the
process,including venting and cleaning,is completed. Dispatch center telephone number for this
