I have chosen the following Alternative as a method of disposing of the diseased/hazardous tree(s) on my property. 1. I will remove the tree within 30 days from the date of notice and be responsible for any cost of removal. 2. I request the City to remove the tree and bill me for the cost of removal. I will pay the City for the cost within 30 days of receipt of a billing. 3.
