As requested, this letter shall document PAI's acceptance of the work completed at the above noted
address per construction documents dated 12-8-14 prepared by PAI.
conducted on-site observations twice prior to the concrete pour, on 1-15-15 and 1-16-15. On 1-16-15, 1
approved the slab rebar in place. See the attached inspection report for that date.
I also have reviewed the attached concrete delivery ticket which was provided to me by you. The mix
design shown appears to be in compliance with the specification.
Therefore, to the best of my knowledge, the contractor complied with the specification for this project.

1-16-15: Project Status: Rebar in place for garage structural slab pour today.

1. I observed the rebar in place. All rebar was acceptable for quantity, size, spacing and cover. All rebar was in place
previous conversations and direction from me during site visit on 1-15-15. All rebar in place was epoxy coated.
Final chairing was needed in a few locations that I pointed out to the contractor.
2. Steel beam wl studs was in place. Bearing length on CMU walls was adequate each end.
3. This field report is the final acceptance by PAI of the work in place.

I have attached the engineers plans for the garage slab poured at 15 Rice Creek Way NE. The engineers name is;
Mike Diess, Office# 763-533-9403, Cell# 218-820-7534. The contractor that put in the floor is; John Couillard, Cell#
952-657-3225. 1 talked to John and he told me you may contact him or Mike with any questions you may have
regarding garage floor. My cell # is 651-900-2474. 1 hope this will resolve the issue of not obtaining the permit in

146-15: Project Status: Rebar in place for garage structural slab pour today.

1. 1 observed the rebar in place. All rebar was acceptable for quantity, size, spacing and cover. All rebar was In place
previous conversations and direction from me during site visit on 1-15-15. All rebar in place was epoxy coated.
Final chairing was needed in a few locations that I pointed out to the contractor.
2. Steel beam w/ studs was in place. Searing length on CMU walls was adequate each end.
3. This field report is the final acceptance by PAI of the work in place.
