Dear Mr. Ficek:
A final inspection was made on December 14, 1977 of the above address and
the following items were noted which must be completed before the structure
can be f inaled out;
1. Provide sod in the front and side yards by June 9, 1978.
2. Provide steps on west exit or render door inoperable by December 25, 1977.
3. Firestop garage wall on the west side by December 25, 1977.
4. Reduce deck spacing to 9 inches by June 9, 1978.
S. Repair hole in block wall near meter by June 9, 1978.
6. Warped siding on the east side should be repaired by June 9, 1978.
We will expect the above items to be completed by the scheduled dates when
reinspections will be conducted to determine compliance.

Contractor: Standard Heating & Air Conditioning Co.
Dear Mr. Garrett:
A mechanical permit was issued on August 28, 2003 to install an air conditioner at your
address. According to the 1991 Uniform Mechanical Code a final inspection shall be
conducted on the work authorized by this permit. As of this date the inspection has not
been requested.
The permit fee that was paid covers the inspection to make sure the work was completed
according to the Codes. We will keep your permit open for another 30 days and if we do
not hear from you within this time to set up the inspection, we will mark the permit "no
inspection called for by homeowner after notification" and the permit will be expired.
