Dear Mr. Roelofs:
I was contacted by Mrs. Johnson of 6388 Pierce Street N.E. this morning.
who indicated that she had a problem with squirrels entering her attic
through a hole in the roof. Our records show you to be the contractor
for the addition work that was done. No final inspections were called
for.
After inspecting it today, I find that the roofing was not installed
with proper flashing and that because of this, there exists a hole for
rodent access.
Please contact me on Monday to discuss corrections that are required
at 571-3450, between 8:30 A.M. and 5:00 P.M.

This form is offered to meet the abandonment requirements of Minn. R. 7080.2500 and Disclosure Requirements of Minn. Stat.
§ 115.55, subd. 6. Future water supply well placement can also be affected by an abandoned SSTS.
The use of this form is not mandatory; however the information on this form must be submitted to the local government unit (LGU)
within 90 days after the abandonment. This form may be completed by a certified SSTS practitioner or by an individual who has
