Dear Mr. Don Holien:
A mechanical permit was issued on December 17, 2003 to install two furnaces at your
address. According to the 1991 Uniform Mechanical Code a final inspection shall be
conducted on the work authorized by this permit. As of this date the inspection has not
been requested.
The permit fee that was paid covers the inspection to make sure the work was completed
according to the Codes. We will keep your permit open for another 30 days and if we do
not hear from you within this time to set up the inspection, we will mark the permit "no
inspection called for by homeowner after notification" and the permit will be expired.

of the intense storms during construction. The date of the incident was Saturday, August 3, 2002.
1 have included one copy of the project Specifications which contains the bid prices and contract at the back. The
contractor for the project was W.B. Miller inc., 6701 Norris Lake Road, Elk River MN 55330. The contract was
awarded at the May 20, 2002 City Council meeting. Construction commenced on June 10, 2002. The project was
substantially complete by the end of October and Final Completion and Payment was approved at the December 9,
2002 City Council meeting.
Also included is a page from the Minnesota Pollution Control Agency's (MPCA) Stomrwater Best Management Practices
Manual pertaining to Storm Drain Inlet Protection. The requirements for storm water control for Construction Activities
by the MPCA are quite expensive. 1 recommend you review their website for the most comprehensive information.

The Rice Creek Watershed District is the local governing unit in charge of storm water controls for the area. They also
issued permits to the City of Fridley for this project and require inlet protection and other best management practices
to prevent construction sediment from being discharged into Locke Lake.
The storm sewer system was designed and installed with the original development of the area in 1959 and 1960. Our
project did not change anything in regards to storm water drainage or capacity. What we did install was a storm water
filtration device to prevent trash and sediment from entering the fake through the storm sewer system. This device has
two manholes and that is the second manhole Mr. Holten is referring to. The storm water treatment device was
Installed on August 8 and 9, 2002.
