Thank you for the narrative and survey. | think| have good news for you! After our staff meeting this morning vve
determined that your use would not require aspecial use permit. VVewould consider your use more ofa "group
home" use as opposed to a "nursing home" use and therefore,a special use permit isn't required in City code. You
will however need to make sure you have the necessary permit from Anoka County and the State of Minnesota.

I will be requesting a return of your application fee to the City's Finance department this week, so I would look for
that check in the mail in the next few weeks.
Please note, as you stated in your narrative, building permits will be required for any building modifications. There is
also a need for a dumpster and dumpster enclosure on this site. We've had many issues with the "residential
garbage carts"that the current owner has used for this building because they can't hold the garbage capacity for
this size of a building. So in your modifications to the site, please make sure you have ample room for a garbage
dumpster and recycling containers. These items will need to be enclosed so as to not be visible from the right-of-
way.

It was a pleasure to meet you in person this morning. As promised, below is the narrative that describes how
my company intends to utilize the building located at 5401 4th Street NE. Additionally, I am attaching a word
document that includes the same description for your convenience.
Explanation of property use:
Empowerment Healthcare is a provider of Home and community-based services. We offer Community Access
for Disability Inclusion (CADI) to our clients. We provide some nursing services and daily living help to our
clients. Home and community-based services are necessary to CADI clients to promote optimal health,
independence, safety and integration.

We are purchasing the small 4 units apartment building located at 5401 4th Street NE to run our Home and
community-based services program. Our program will include people with chronic diseases such as diabetes,
high blood pressure, COPD, etc. Our program help our clients manage their conditions in order to live
independently. Sex offender and people with substance abuse addiction will not participate in this program.
Empowerment Healthcare clients' will not have vehicles and will use a company van for their transportation
needs. Given that there will be 2 employees at the most in the property; there will be more than enough
parking space in the 8 parking area located behind the building to accommodate our parking needs.
We will fully renovate the building (which is in poor condition) after getting the permit from the city of Fridley.
The renovation will include replacement of all windows (new windows), replacement of all floors, changing all
appliances, repainting the all inside walls, redoing the electrical panels, and more.

Per Minnesota Statute 15.99, local government units are required to notify land use applicants within 15
working days if their land use applications are complete. We officially received your application for a
Special Use Permit for a group home at 54014 th Street on March 18, 2016.
After further review, staff determined that a special use permit is not required for the use you are
proposing for the property at 54014th Street. Based on your description of the use, the City would
consider it a group home instead of a nursing home, clinic or hospital use. Group Homes in residential
districts are regulated by Anoka County and the State of Minnesota, so provided you have the right
licenses in place from them, a special use permit from the City isn't required.
I have initiated the process to refund the special use permit fee you paid to the City. You should receive
a check in the mail within the next 2-4 weeks.
