BUILDING PERMIT FEE SCHEDULE
SECTION 2. The Inspector of Buildings shall, before issuing permits for the
erection of any building or structure, or for any addition to any existing
building or structure, or for any alteration or repairs to any existing
building or structure, upon application therefore, require the payment by the
applicant for such permit of fees to the amount herein below set forth and in
the manner herein provided to -wit:
UNIFORM BUILDING CODE: Type 1, 2 & 4 - $2.15 for each one thousand cubic
feet, or fraction thereof, in such cubical contents. Type 3 & 5 - $1.85 for
each one thousand cubic feet®
ADDITIONAL INFORMATION
For the purpose of computing fees for building permits, the cubical contents
of any building or addition, is determined by multiplying the ground area
covered from a point six (6) inches below the floor line of the basement
or the cellar to the average heights of the upper surface or to the average
height of the roof surface of the main gable of a pitched roof.
For repairs or alterations to an existing structure, the fee shall be at the
rate of $3.00 per each five hundred dollars ($500.00) or fraction thereof in
the cost of all proposed work.
In no case shall the fee charged for any permit as set forth in Section 2 be
less than $5,00,
VERIFICATION OF FOUNDATION
NOTE; Permits for construction will be issued a minimum of 24 hours from the
time of application, to allow for proper review of the proposed structure and.
of the construction site,
A Certified Survey of the lot, showing the location of the foundation, once it
has been constructed, will be required before Proceeding with the framing.
CERTIFICATE OF OCCUPANCY
Application for a Certificate of Occupancy shall be made ten (10) days prior
to the use or occupancy of any structure for which a building permit has been
issued; and said structure shall not be used or occupied until a Certificate
of Occupancy has been issued.

As you requested, a soils investigation has been made on the
above referenced lot toassistin evaluating the foundation --
conditions ' for a proposed house.
The borings were taken at the locations shown on the attached
sketch. These locations were referenced to the apparent property
line along the easterly side of the lot. Surface elevations were
referenced to an assumed bench mark at the centerline of Sylvan
Lane north of the west property line. This elevation was assumed
to equal 100.0.
The borings were taken on June 29, 1973, with a truck -mounted core
and auger drill equipped with hollow -stem augers. Penetration
test samples were taken through the hollow -stem augers with a
standard 2 -inch OD split sampler driven by a 140 -pound hammer
falling 30 inches in accordance with ASTM D1586 "Penetration
Test and Split.Barrel Sampling of Soils". Blows per foot of
penetration (BPF), which are an index of the various soil strength
parameters, were recorded. The bore holes were cleaned and advanced
between samples with continuous flight, hollow -stem augers
eliminating the.driving of casing and the use of jetting water.

Mineral soils encountered in the borings were visually and manually
classified in the field in accordance with the U.S. Bureau of
Chemistry and Soils Classification System. A copy of that chart is
attached. A few representative samples will remain in this office
for a period of 60 days to be available for your examination.
Each boring was probed immediately after completion to check for
the presence of ground water. The borings were then rechecked
prior to leaving the site and were then backfilled.
RESULTS
Logs of the soils encountered, penetration resistances, and water
level observations are shown on the attached LOG OF BORING sheets.
The borings generally encountered granular soils to an approximate
depth of 14 feet. At that depth a slightly plastic to plastic sandy
loam was encountered. In boring ST -2 the upper 2 feet of the
soils appear to be fill placed over a thin layer of black soils
which were probably the original topsoils.
Penetration resistances in these granular soils ranged from a
minimum of 6 BPF to a maximum of 15 BPF indicating a loose to
medium dense condition. Penetration resistances in the underlying
cohesive soils ranged from 8 to 12 indicating a medium to rather
stiff condition.
When probing the borings immediately after completion no ground
water was encountered. However, when the borings were rechecked
prior to leaving the site, water was encountered at the 14 -foot
depth in ST -1 and at the 13.5 -foot depth in ST -2. Judging from
.the moisture content of the samples obtained it appears that this is
the approximate elevation of the water table at this time. Water
levels should be expected to show normal seasonal and annual
fluctuations.
RECOMMENDATIONS
You have indicated that you propose: to build a one-story rambler
with a full -depth basement and a rear walk -out. From the elevations
of the street in front of the lot, it appears that the finished
grade on the outside of the house in the front would be at an
approximate elevation of 103 on the assumed datum. This would
result in a basement elevation of approximately 95. Thus, the
front footings would .be at an approximate elevation of 94'and
the rear footings would be stepped down to an approximate elevation:
of 91 on the assumed datum.

The footings at the locations -of the borings will be in fine or
fine to medium sands which are in a loose to medium dense condition.
These sands would easily be capable of supporting the loads
associated with a typical house on minimum -width footings. While
the front part of the house was not investigated, it is probable
that similar soils will be encountered throughout. However, this
should be verified by inspection prior to the placing of footings
or floor slabs as significant variations in soils can occur.
The water table appears to be currently 9 to 10 feet below the
assumed elevation of the basement floor slab. It is suggested that
you consult with the adjacent home owners to determine if there
are wide fluctuations in ground water table which would suggest
a necessity for foundation draintile.
Of necessity, the area of the borings in relation to the area of
the lot, and the depth of the borings, are limited. Suggestions
and/or recommendations of this report are opinions based upon
the data obtained from the borings.

DRILLING NOTES:
Standard penetration test borings were advanced by 2%" or 3%" I.D. hollow -stem augers unless noted other-
wise. Jetting water was used to clean out auger prior to sampling only where indicated on logs. Standard
penetration test borings are designated by the prefix "ST" (Split Tube).
Power auger borings were advanced by 4" or 6" diameter, continuous-flite, solid stem augers. Soil classification
and strata depths are inferred from disturbed samples augered to the surface and are therefore somewhat
approximate. Power auger borings are designated by the prefix "B".
Hand probings were advanced manually with a 1%" diameter probe and are limited to the depth from which
the probe can be manually withdrawn. Hand probings are indicated by the prefix "H".
CLASSIFICATION:
Classification on logs is made by inspection in accordance with the U. S. Bureau of Soils Classification
System (see attached chart) using visual -manual procedures unless noted otherwise.
GROUND WATER:
Observations were made at the times indicated. Porosity of sail strata, seasonal weather conditions, site
topography, etc., may cause changes in the water levels indicated on the logs.
SAMPLING:
All samples are taken with the standard 2" O. D. split -tube sampler, except where noted. TW indicates thin-
wall undisturbed sample.
BPF:
Numbers indicate blows per foot recorded in standard penetration test, also known as "N" value. The sampler
is set 6" into undisturbed soil below the hollow -stem auger. Driving resistances are then counted for second
and third 6" increments and added to get BPF. Where they differ significantly, they are reported in the
following form — 2/12 for the second and third 6" increments respectively.
WH:
WH indicates that sampler penetrated soil under weight of hammer and rods alone, driving not required.

Plumbing permits were issued on November 20, 2000 to install a water heater, and on May 25,
2001 to install three fixtures at your address. According to section 4715.2800 of the State
Plumbing Code, a final inspection shall be conducted on the plumbing authorized by these
permits. As of this date, no inspection has been called for.
The permit fees that were paid cover the inspections to make sure the work was completed
according to the Plumbing Code. We will keep your permits open for another 30 days and if we
do not hear from you within this time to set up the inspection, we will expired the permit and no
further action will be taken.
