A mechanical permit was issued on January 13, 1998 for the property located at 5816 Tennison
Drive NE for a furnace and air conditioning replacement installation. According to Section
305(a) of the 1991 Uniform Mechanical Code a final inspection shall be performed on the
mechanical system. As of this date no inspection has been called for.
You will be given thirty (30) days from receipt of this notice to respond. If we do not hear
from you, we will expire the permit and any inspections on this installation will require a new

Guest Instructions:
1. Take this picking list to a cashier to pay for the merchandise.
L Enter the outside yard to pick up your merchandise. (All vehicles are subject to inspection.) PRE-TAX TOTAL: ( CONTINUED )
3. Load your merchandise. (Menards Team Members will gladly help you load your materials
but cannot be held liable for damage to your vehicle.)
4. When exiting the yard, present this list to the Gate Gnarl. (Che Gate Guard will record the
Items you are taking with you.)
S. Sign the Gate Guard's signature pad verifying you've received the merchandise.
Our insurance does not allow us to tic down or secure your load, trunk lid, ate. For your convenience, we supply twine, but you will have to decide whether or not your
load is secure and if the twine supplied is strong enough. If you do not believe the twine will suffice, stronger anterial can be purchased inside the store.

that if any merchandise purchased by you is defective, Menards will agree to exchange the merchandise or refund the purchase price based on the form of original payment.

or using it. UNDER NO CIRCUMSTANCES SHALL MENARDS BE LIABLE FOR ANY SPECIAL, INCIDENTAL, OR CONSEQUENTIAL DAMAGES.
MENARDS MAKES NO WARRANTIES, EXPRESS OR IMPLIED, AS TO MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE OF THE
MERCHANDISE. Any controversy or claim arising out of or relating to this contract, or the breach thereof, shall be settled by arbitration administered by the American
Arbitration Association under its applicable Consumer or Commercial Arbitration Rules, and judgments on the award rendered by the arbitrator(s) may be entered in any
court having jurisdiction thereof. The guest agrees to these terms and conditions through purchase of merchandise contained on this document.
