SECTION 2. The Inspector of Buildings shall, before issuing permits for the
erection of any building or structure, or for any addition to any existing
building or structure, or for any :41teration or repairs to any existing
building or structure, upon application therefore, require the payment by the
applicant for such permit of fees to the• amount herein below set forth and in
the manner herein provided to -wit:
UNIFORM BUILDING CODE: Type 1, 2 & 4 -• $2.15 for each ons: thousand cubic
feet, or fraction thereof, in such cubical contents. Type 3 & 5 - $1.85 for
each one thousand cubic feet.
ADDITIONAL INFORMATION
For the purpose of computing fees for building permits, the cubical contents
of any building or addition, is determined by multiplying the ground area
covered from a point six (6) inches below the floor line of the basement
or the cellar to the average Heights of the upper surface or to the average
height of the roof surface of the main gable of a pitched roof.
For repairs or alterations to an existing structure, the fee shall be at the
rate of $3.00 per each five hundred dollars ($500.00) or fraction thereof in
the cost of all proposed work.
In no case shall the fee charged for any permit as set forth in Section 2 be
less than $5.00.
VERIFICATION OF FOUNDATION
NOTE: Permits for construction will be issued a minimum of 24 hours from the
time of application, to allow for proper review of the proposed structure and
of the construction site.
A Certified Survey of the lot, showing the location of the foundation, once it
has been constructed, will be required before proceeding with the framing.
CERTIFICATE OF OCCUPANCY
Application for a Certiiicate of Occupancy shall be made ten (10) days prior
to the use or occupancy of any structure for which a building permit has been
issued; and said structure shall not be used or occupied until a Certificate
of Occupancy has been issued.

Dear Mr. Dostaler:
The City continues to receive complaints from the two home owners at
5701 and 5703 Washington Street N.E.
Mr. James Proctor at 5703 Washington Street N.E. informs me that the
roof repair still has not been corrected. The problem here is that
the shingles and starter roll are short and do not extend behind the
edge of the roof. This definitely must be corrected and should be
done as soon as possible, therefore in order not to let this drag on
over a period of weeks, I will state that this correction must be
made by August 4, 1975.
The owner, Mr. Frank Sasek, at 5701 Washington Street N.E. has reported
to me quite a long list of problems he has brought to our attention,
the more serious of which are:
1. Tile on basement step loose - not enough contact
cement used.
2. Roof leak near furnace flue.
3. Siding on north wall of house is buckling.
4. Nuts and washers on bottom plate in garage missing.
5. Squeaking floors at various locations.
6. Complaint on interior paint being rough.
Would you please either correct these problems or report to ]Re and
the owner why you are not going to remedy the various complaints.
I will expect to hear from you by August 4, 1975 in regards to these
matters.

Roof Repair
Dear Mr. Dostaler:
This letter is with reference to the repair to the roofing at
the James Proctor residence at 5703 Washington St. N.E. that
you reported completed.
On Tuesday, July 29, 1975, we made an inspection on the repair
work and found the method used to be inadequate and unacceptable.
The attempted correction was made with rolled asphalt and will
not prevent future backup that is sure to occur during.the winter
months.
A different method must be used and I suggest that you have
your roofer contact me so that we can discuss the method to
be used in making the correction.
This matter must be corrected and I feel that a one week extension
to the August 4, 1975 date set in my letter of July 15, 1975 is
reasonable.
Your cooperation in this matter will be appreciated.

RE: 5703 Washington Street N.E.
A meeting of Virgil Herrick, Mr. & Mrs. Proctor, Ivan Dostaler, Albert Rosenbower
and Darrel Clark in City Hall at 2:00 P.M.; the purpose of the meeting was to
settle the defects in the construction on the Proctor home located at 5703
Washington Avenue constructed by Mr. Ivan Dostaler.
I. Roofing: Asphalt shingles are short of the bottom of the roof.
The contractor agreed to remove the lower five courses, call for an inspection,
then relay the bottom five courses to provide for the proper extension over the
end of the roof. Also at that time the rest of the roof will be examined for
imperfections.
Special attention should be given to determine cause of the apparent leak
in the kitchen area. This work must be completed on or before May 3, 1976.
II. Driveway Settlement:
1. The driveway developed a hble which was repaired and which now has sunk
again. The probable cause is an improperly filled cesspool. The contractor
has agreed to daylight the top of the old cesspool and call for an inspection
and then compact as deemed necessary to subgrade patch the driveway and seal
the entire driveway so the patch is no longer visable.
This work is to be completed on or before June 15, 1976.
III. A.Basement block shows signs of apparent leaks:
1. The probable cause is poor drainage away from the foundation. The City will
offer an opinion about this after first visiting the house this spring,
and if grading is improper or if settlement has occured adjacent to foundation,
the Contractor has agreed to regrade and resod in the areas disturbed by
regrading. The contractor has also agreed to supply paint to repaint the
basement walls that have water stains.
B.Contractor has agreed to supply paint for two bedrooms that have water
stains due to the roof leaks.
This work is to be completed on or before June 15, 1976.
IV. Bathroom tile in basement to be repaired on or before May 3, 1976.
V. The Building Code will be reviewed as to the necessity for metal window wells.

REGARDING: 5703 Washington Street N.E.
Since April 1, 1976 seven inspections have been made by City
personnel at the above address. These inspections were made
with reference to the roof and driveway repair work. The one
made on June 15, 1976 was made to determine if all the items
mentioned in the March 11, 1976 report had been completed.
The following constitutes the status of those items;
1. Roofing; the roofing has been completed.
2. Driveway settlement; the driveway settlement has been
excavated, compacted, and refilled and the patch is
complete. No seal coat has been applied to date.
3. Basement block discoloration; nothing has been done
and no regrading is apparent.
4. Bathroom tile; nothing has been done.
5. Window wells; no Code violation.
