When discussed onsite, you stated you would fill out a permit application the following day. Based on our records a permit for the completed work has not been issued. The City will enforce code violations relating to your unpermitted work. The standard fees of $50.00 for the repair permit, $40.00 for inspection and a $1.00 MN State surcharge will apply. Per City Code 206.04, an investigation fee for your violation will be applied equal to the permit fee of $50.00. 'Four contractor license expired April 30, 2017, therefore you will need to renew and pay the license fee of$35.00. Upon visiting the unpermitted work site, it was documented that up to 7-feet of curb was without foundation support. The need for foundation support is outlined in the City's Construction Requirements for Water and Sewer Work. These general guidelines have been adopted in an effort to protect public infrastructure from damage. Engineering staff will monitor the curb adjacent to 57th Place NE for settlement and may enforce a replacement or degradation fee as determined by the City(Fridley Code 407.11). Additionally, as required by the permit the excavated area is to be stabilized within 7 days after completion of the work. Stabilization has not been accomplished to date. At minimum, the City will require you to place an erosion control mat or spread a layer of straw in an effort to prevent exposed soils from eroding off-site. Pipe inspections include but are not limited to connections to existing service pipe,connections to City owned pipe or manholes,tees,valves,curb stops,corporations,wet taps, bends,mechanical restraints and hydrants. Protect City infrastructure facilities(curb,manhole,pavement,striping,etc)from damage and loss of foundation support. Loss of foundation support is identified as excavation within the 1:1 slope from the bottom of the structure. Surface damage or loss of foundation support shall result in replacement of facility or degradation fee as determined by the City(Fridley Code 407.11). Backfill material shall be compacted in 1 foot lifts(gross)using a mechanical compactor. Upper 4'of utility trench sides shall be excavated and compacted at 1:1 slope unless engineered shoring is installed prior to excavation and approved by the City. Trench sides deeper than 4' exhibiting loss of lateral density(i.e.cave in,sloughing,undermining,cracking,etc)shall be excavated and recompacted as approved by the City. The City may consider a 5 year bond in lieu of the additional excavation and compaction required to achieve uniform density.Contractor shall comply with current OSHA excavation provisions and CEAM Standard Utility Specifications(2013 Ed.).All new residential and commercial construction to meet with the City prior to construction. Horizontal directional drilling shall require the televising of other pipe in the vicinity to verify no damage has occurred. Excavations within ROW require density of backfill materials to be 100%in the upper 3' and 95%below the upper 3'. Density shall be verified by one of the following methods as allowed by the City: • City approved soil testing company(2 test minimum)or; • Granular soils may be flooded with 2' of water twice(5' and 2' from grade)or; • Visual observation during backfill by City(hourly cost of$75 to observe during 8am—5 pm,M-F). Reconstruct pavement patch area: • Sawcut pavement as marked by City—contractor to coordinate marking with City • Residential street-6"Cl. 5 aggregate and 3"LV4 bituminous asphalt(2 lifts) • Non-residential street—8"Cl. 5 aggregate and 4"MV3 bituminous asphalt(2 lifts) • Match existing patch sections if greater than the above minimums. Provide traffic Control per MMUTCD. If 2-way vehicle travel(22' minimum width)is not maintained,contractor shall provide a MnDOT certified flagger to direct traffic or provide detour as approved by the City.If work impedes a pedestrian travelway(i.e. sidewalk,bike path,etc.)Contractor shall provide a pedestrian detour route in accordance with ADA rules and regulations. Sewer Non-gasketed pipe connections shall use a Fernco 5000 Series Strong Back RC Coupling where applicable.Contractor shall use a Fernco Eccentric Series Coupler for differing pipe diameters.Flow lines shall be maintained level and continuous by aligning the inside walls of the two pipes. Single band Fernco couplings to have a 12"x12"xl"minimum concrete base for support. CIDP(cure in-place pipe lining)must meet the minimum ASTM standards:ASTM 1216,ASTM D-638 and ASTM D-790.Minimum thickness of liner to be 4.5 mil.The service line must be cleaned,televised and recorded before and after installation.No protrusion of liner into the City sewer main is acceptable.A post-installation video shall be submitted to the City.If a copy of the video is not available an inspector will view the televising of the new liner with the Contractor.Televising must be completed before the installation is approved.Pipe bursting or horizontal directional drilling work requires a plan showing existing utilities and location of installation pits.Contractor shall meet with a representative from the City Engineering Department before beginning work. Disconnection of service pipe shall be plugged at property line with a Fernco Qwik cap and marked with a 4"x 4"post. All Rine shall be removed within the property unless otherwise approved by the City_ Water Flaring existing copper is not allowed.Contractor shall silver solder or use pack joint coupling compression fittings to make connections to existing copper services.Contractor shall notify owner of inoperable curb stop requiring repair(Fridley Code 402.06). Approved repair materials are as follows: Copper Type K—ASTM B 88,PEX ASTM-F876/877 or Endol'ure PE-3408-200 PSI Disconnection shall typically be on building side of curb stop. Contractor shall leave a 1' stub and silver solder the end closed.Curb stop shall be opened fully for 5 minutes and checked for leaks by the City. Contractor shall correct any leakage of the curb stop or 1-ft stub and retest. All pipe shall be removed within the property unless otherwise approved by the City. Erosion Control Disturbed soils shall be stabilized within 7 days after completion of work. Disturbed vegetated areas shall be backfilled with 4"of topsoil meeting MnDOT 3877.2A,seeded and mulched prior to Contractor leaving the work site.Erosion control measures shall be installed to stabilize and prevent exposed or disturbed soils from eroding off-site or into a waterway. Contractor shall identify erosion control measures to be used on front of permit. Revised 1/17
