Date Inspection Narrative
9/26/08 During
a lockbox inspection, I heard what sounded like a beeping noise coming from the fire alarm/sprinkler/mechanical room at the property address. As I was retrieving the key for that
room from the key box, a civilian told me that the beeping has been going on for the last several days. I opened the door and accessed the fire alarm control panel which monitors
the fire sprinkler system.
Zone 4 was in an active supervisory alarm. The legend on the FACP states that it is a supervisory signal which emanates from the two sprinkler system
gate valves on the building riser which is also in the room. I tested the gate valves and they were fully in the open position. I silenced the alarm and attempted a reset of
the system. There was no indication electronically or audibly that a flow alarm had been tripped. When I returned to the office, I called Bob Cote, owner of Fridley Crossing and
received no answer.
9/29/08 I reached Bob Cote concerning the supervisory alarm which was active at his property. He stated that his fire alarm company is Wellington Security and
the system is monitored by USA Security. He has received no calls from the alarm company reporting a supervisory alarm involving any gate valves. To me, this indicated a possible
malfunction of the fire alarm system and the tamper devices. I contacted USA Security from the property and they had no indication of a supervisory gate valve signal being transmitted
from the property. I contacted Mr. Cote again and let him know what I discovered and he stated he would contact Wellington to repair the alarm system and test signal transmission
to the monitoring company as soon as possible. He will contact me after Wellington has been at the site. I also offered to meet Wellington on site if they so choose.
