We hereby propose to furnish materials and labor necessary to:
Remove all flue tile liner materials and reline masonry chimney for—fireplace, using 10"round stainless steel Liner
system (U.L. Listed with manufacturer's lifetime limited warranty); remove concrete wash and 10 courses of brick on
chimney; rebuild to current day code requirements using"best match brick"; provide concrete crown with overhang
and drip ledge as outlined in article"TOPS TO LAST A LIFETIME"by Jack Pixley;install a top mount damper in main
level fireplace, repair smoke chamber void.
INCLUDES:
-Ground 8 roof protection
-Scaffold and equipment setup Et removal
-Existing concrete wash, tile, and brick removal
-Access into chimney
-Debris disposal
-Install stainless steel liner Et insulation
-Retile chimney top
-Concrete crown at top
-Install stainless steel rain cover
-Cleanup/landfill/recycle/travel/unload/remove forms
-Building permits Et inspection
Conditions:
All material is guaranteed to be as specified.All work will be completed
professionally and in accordance with the IRC 2006 and manufacturers'
warranty.
If we find a code violation during repair work; We reserve the option to stop
work, show owner and/or consult with the building inspector.This may result
in additional costs for completion of the job.All agreements are contingent
upon weather or delays beyond our control. Our company is fully insured and
employees are covered by Liability and Workman's Compensation Insurance.
Acceptance and Payment Terms:
We require 25%downpayment with signed proposal. The remaining balance is due
upon completion of the described work. Checks shall be made payable to Jack
Pixley Sweeps, Inc. We accept Visa,Master card and Discover with a 3%
processing fee. Finance charges of 0.7942%applied after 15 days of open
invoice due. Invoices that are over 45 days from job completion will be
submitted to collections. (Additional legal fees may be included.)
Discrepancies:
Please notify Jack Pixley Sweeps, Inc. with any deficiencies regarding your
installation within 72 hours so that we may address any concerns regarding
the work.Otherwise,we will assume everything is in satisfactory condition
and acceptable.Winter
Inclement weather delays for masonry repairs are soon upon us. Jack Pixley Sweeps cannot commit to completing your
accepted chimney work before winter. Winter work continues on certain "Winter Friendly"repair jobs. However, you
are welcome to take advantage of the opportunity to get on the upcoming spring repair schedule early. For customers
that choose to wait for spring,we ask only for a$100.00 non-refundable deposit with the remaining 25%down
payment due March 1st. Our repair scheduling is planned on a first come first serve basis.

provide the goods and services specified in this specifications and conditions are satisfactory and are
proposal. 1 agree to all terms specified in this hereby accepted. You are authorized to do the work
proposal.This proposal can be withdrawn by us after as specified. Payment will be made as follows; 25%at
30 days of being sent. acceptance and remained at completion of job.
