Electrical Release
Addendum to Contract
This document is in reference to a contract agreement dated between the following parties that are
named below in this document.
May it be known that &0 . � %e4q tn S , the undersigned party, for good
consideration does hereby agree to hire and work directly with my/our own electrician during the contract agreement
with Budget Exteriors, Inc . In accordance to Minnesota State Law and the Department of Labor & Industry, an individual
performing electrical work must be either licensed or registered . Budget Exteriors is not a licensed electrician .
The re-installation of existing luminaries, devices and equipment as well as the installation of new electrical wiring,
junction boxes, new luminaries, devices and equipment shall be performed by licensed or properly supervised registered
electricians who are employees of licensed electrical contractors and the work shall be inspected . Alternatively, the
electrical work may be performed by the homeowner who performs the electrical work on the dwelling that they own
and occupy .
Whether the electrical work is performed by the owner or by the employees of an electrical contractor', a valid request
for an electrical permit and inspection shall be filed with the authority having jurisdiction — The Department of Labor &
Industry or the city — before any electrical work begins .

undersigned stated contract with Budget Exteriors, agrees to pay the balance of
said contract understanding that the completion of the contract dated has no bearing on the electrical
work necessary or needing to be completed .

No other terms or conditions of the above mentioned contract shall be negated or changed as a result of this here
stated addendum .
