NE Fridley, MN 55432 RE: ROD23-000040— Driveway Permit Violations Mr. Avila, The City of Fridley has completed a site inspection of the work performed under this permit and found the following items that will require correction prior to release of your escrow: • Expansion Joint is required between the curb and the driveway apron. Integral curb/aprons are not allowed. Provide an expansion joint at the backside of curb per city standard details included as part of the permit application (enclosed). • Gutter and street at area of new curb shows signs of damage. Provide new concrete curb and gutter with a 2' street patch per city standard details and requirements noted here: o All openings made in existing bituminous surfaces shall be sawcut, full depth, in a straight line with edges smooth and vertical. No ragged edges will be permitted. o Base and Pavement course of patch to match existing adjacent coursing. o See enclosed detail for curb and gutter requirements. • Sanitary manhole appears to be partially covered in concrete. Ensure manhole is free from concrete and can be opened without issue. Contact City to open manhole after concrete/debris has been removed for inspection. Any cost incurred by the City above and beyond the current escrow amount will be the responsibility of the homeowner. Operations shall be limited to 7am to 7pm and shall not interfere with the health and safety of surrounding residents and the premises shall be maintained at all times so as not to create a nuisance. 2. At the end of each day's operations, the site is to be left in a neat and orderly condition, and all adjacent streetsswept clean of dirt. 3. Curb placed in City ROW must meet the requirements of the attached standard detail. 4. The Contractor completing the work must be registered with the City of Fridley. Call 763-572- 3604 for more information on registration. 5. The Contractor, or Owner, will call the City at 763-572-3554 to arrange for an inspection of the forms prior to placing concrete or asphalt on site. Twenty-four hour notice is required for inspections and will be provided between 8:30 am and 2:30 pm Monday through Friday. 6. Repair to the bituminous surface on the roadway side of the curb will be the responsibility of the owner/contractor. Bituminous pavement removed will be saw cut and the replacement area sub-cut two feet and filled with class V aggregate compacted in six inch liftsto 95 percent standard proctor density. Sub-cut area must be inspected by City of Fridley prior to placement of class V aggregate. Call 763-572-3554 to schedule the inspection. Bituminous patch will match the existing pavement, or a minimum of 3 inches compacted wear course bituminous. 7. Erosion and Sediment Control is required during work. Streets and gutters are to remain free of dirt at all time. Street sweeping required each day and all disturbed areas planted with suitable vegetation and protected to preventerosion. 8. Upon completion of paving operations or expiration of this permit, an inspection will be made by the City of the premises and adjoining streets. Any damage found to have been caused by these operations will be corrected by the applicant upon notification by the City. Note 2 :Commercial Driveway requires Land Alteration Permit. Note(3): Escrow to be returned if work is completed according to the permit requirements. If City is required to complete, repair or replace any of the work,the escrow will be forfeited.
