The City Council of the City of Fridley does hereby orclain as follows:
206.03. �'�'� FEESS.
2. The fee schedule shall be as follows:
S. Monitoring Well Permit Fee
1. Each ground water monitoring well location shall require a site
map, well description and length of monitoring time requested. (For
matter of de£inition monitoring well location shall n�i each
individual tax parcel.) There shall be an initial application and
plan check fee of �aenty Five Dollars ($25).
2. An initial monitoring well permit fee of Fifty Dollars ($50) is
due and payable to the City of Fridley at or before coimnencement of
the installation.
3. An annual permit fee of Fifty Dollars ($50) and annual
monitoririg aetivity reports ior all individual monitoring locations
� must be made on or before September first of each year. If
registration is not filed on or before october first of each year
the applicant must pay double the fee.
4. A final monitoririg activity report must be submitted to the City
within (30) days of term;nation of monitoring well activity.
5. A copy of all monitoring well permits, annual reports and final
reports shall be submitted to the Minnesota Department of Health by
applicant at the time the same is submitted to City.
